Chemonucleolysis with Chymopapain


Abstract
During the thirteen-month period from December 1, 1984, through December 31, 1985, chemonucleolysis was performed under the local anesthesia in 24 patients upon the diagnosis of herniated nucleus pulpous, 14 men and 10 women, ranging in age from 17 to 63 years with the average age of 36.4 years, and the results were reviewed.
1. Out of 24 patients excellent or good results were obtained in 19 patients (79.2%) and a poor result in one patient (4.2 %) .
2. The frequencies of the lesions were 16 cases(66.7%) at L4-5 intersperse, 4(16.6%) at L5-S1, 1(4.2%) at L3-4 and 3 patients (12.5%) had multiple lesions.
3. The male to female ratio was 1.4: 1 and 18 patients(75%) ranged in age from 20 to 40 years.
4. No serious complication was encountered, but 10 patients(41.7%) experienced minor adverse reactions that regressed spontaneously within a few days.
5. The most important thing in chemonucleolysis is the selection of the patient. Excellent or good results were obtained in those adults of less than 50 years of age who had typical posterolateral and unilateral disc herniation of short duration compressing only one spinal nerve.
